I am medically fit to participate in dance classes conducted by LVDS.

If the above answer is a no, please detail your medical condition. PLEASE NOTE: You may have to bring in a NOC from your Physician.

LVDS can use my email id and phone number to contact me with respect to any class/workshops/socials/events and dance related information unless expressed explicitly by me.

I will not hold the directors, trustees, advisors, volunteers, instructors, employees or any other individual or enterprise responsible for any loss or damage to life or property.

I authorise the LVDS team to use my pictures and videos in any form for promotion in all media

All fees paid are non-refundable and non-transferrable.
